About The Position

Provides education, acts as a liaison with the patient, physician, family, other health care personnel, hospital administration, agencies and community groups throughout the patient's hospital stay or service. Supports and guides patient and family in the health care decision-making process including but not limited to admissions, diagnostics, surgical/medical interventions, discharge, and support services. Serves as point of contact, clinical leader and resource to staff and administration. Provides patients and family with needed information for the transition from active treatment to managing their condition independently. Patient care may involve inpatient and outpatient services. Patient care needs will vary depending on the service line area. Responsible for performing job duties in accordance with mission, vision and values of Tampa General Hospital.

Requirements

  • Bachelor's Degree Nursing
  • Florida Registered Nurse License
  • Three (3) years related work experience required.

Responsibilities

  • Serves as liaison for patients and families through all related services.
  • Responsible to continually assist patient/family through the healthcare system which includes during procedures/admissions at TGH, follow up, and with community resources.
  • Demonstrates current knowledge and level of expertise required to navigate the healthcare system.
  • Maintains databases for the program.
  • The ability to gather, analyze, and interpret statistical data.
  • Demonstrates the ability to prioritize and organize patient flow and responsibilities.
  • Lead efforts to achieve national program recognition, and responsible for data collection, analysis and performance improvement.
  • Works with healthcare providers to offer solutions for the patients or helps to coordinate services and scheduling.
  • Uses critical thinking skills to assess patient and staff situations, determining urgency and priority, and acting to resolve or refer as appropriate.
  • Assists with efficient use of TGH resources as measured by LOS, discharge time, charges, etc.
  • Demonstrates a cooperative attitude while interacting with co-workers, personnel from other departments and external agencies to achieve departmental goals and satisfactory patient outcomes.
