About The Position

This specific new position request is budgeted for 2026 for 1.0 CPP FTE to provide both onsite and virtual medication management services, directly addressing current service gaps and supporting quality care expansion in Guilford and Rockingham (14 primary care clinics and 2 specialty clinics). The Ambulatory Clinical Pharmacist – Pharmacy Connect, Guilford is a Full-Time, 1st shift, Mon-Friday position based at the Winston Campus. This role will impact patient care by overseeing pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. The pharmacist will use effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. They will design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens based on relevant biomedical literature, implement recommendations effectively, and utilize policies and procedures while documenting in the appropriate format. The role involves reviewing and accurately interpreting medication orders and prescriptions, participating in the management of medical emergencies, and actively supporting department medication safety and quality improvement activities, including reporting events. The pharmacist will perform quality assurance and performance improvement of the medication use system, collaborate with the healthcare team to manage formulary adherence and the medication-use process, and assist in the acquisition of non-formulary, compassionate use, and investigational medications. Practice must comply with organization’s policies and procedures, as well as local, state, and federal regulations. The role also includes preparing, dispensing, and overseeing the proper storage and handling of medications. Additionally, the pharmacist will serve as a preceptor for pharmacy students, residents, and healthcare learners, and mentor, orient, train, and assist with performance evaluations of pharmacy teammates, residents, and students. They will direct and check the accuracy of the work of pharmacy support team members to ensure safe and efficient workflows, service standards, and regulatory compliance. The position requires the ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, including understanding of growth and development over the life span and assessing data reflective of patient status.

Requirements

  • Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required.
  • Current state pharmacist license is required in good standing through North Carolina Board of Pharmacy.

  • Completion of a PGY1 residency and/or equivalent clinical experience is preferred.
