About The Position

This position primarily serves the Infectious Disease clinic and involves direct patient care and collaboration with other healthcare providers. A strong clinical pharmacy background with an understanding of retail pharmacy operations and prescription processing is optimal to succeed in this role. Cross coverage of other specialty areas may be required. Cencora has managed and staffed the Reading Hospital Ambulatory Pharmacy inside of the 700 bed Reading Hospital since 2017. Since then, an additional 4 retail pharmacy locations were opened to serve the Reading community and outpatient provider offices.

Requirements

  • PharmD degree strongly preferred.
  • Current unrestricted pharmacist license in the appropriate state of practice
  • 5+ years community/retail/ambulatory pharmacy or health-system related experience.
  • Must be proficient in Microsoft Office, including Word, Excel, and PowerPoint
  • Candidates should have prior experience on pharmacy dispensing systems and be familiar with insurance adjudication processes.
  • The role involves working with a broad array of complex disease states and medical conditions; candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records.

Nice To Haves

  • Previous experience in a specialty pharmacy and/or processing prior authorizations is highly desirable.

Responsibilities

  • Demonstrate value of ambulatory pharmacy partnership to physician practices.
  • Maintain presence in clinic, field medication questions from providers, such as formulary product selection, product dosing, ADR's, and other considerations.
  • Ensure prescriptions flow through ambulatory pharmacy and are identified by pharmacy staff.
  • Maintain patient schedule for clinical pharmacist services including, but not limited to, medication counseling, pillbox refills, immunization, medication administration.
  • Maintain patient list, schedule appropriate follow-ups to assess medication safety and efficacy, communicate treatment plan recommendations with provider.
  • Coordinate patient refills. Ensure auditable records showing proof-of-delivery are maintained.
  • Prepare and submit prior authorizations, manage denials/appeals.
  • Triage prescriptions which are limited distribution drugs or out-of-network.
  • Act as financial advocate for patient when needed, leverage charitable foundations, state programs, and/or manufacturer programs to ensure medication affordability.
  • Demonstrate ability to navigate various platforms and document appropriately in EMR and specialty therapy management platform.
  • Contribute to expansion of ambulatory pharmacy services through active participation in navigator committee and projects.
  • Cross-train to ambulatory pharmacy, assist with staffing, as needed.
