About The Position

United Health Services (UHS) is seeking an Ambulatory Clinical Pharmacist to provide comprehensive medication therapy management and disease state management services within the ambulatory care setting. As an integral member of a multidisciplinary healthcare team, the pharmacist collaborates closely with physicians, advanced practice providers, nurses, and other healthcare professionals to optimize medication use and improve patient outcomes. Working under collaborative practice agreements, the Ambulatory Clinical Pharmacist delivers direct patient care through in-person and telehealth encounters, assessing medication regimens for appropriateness, effectiveness, safety, tolerability, and adherence. The pharmacist is responsible for identifying, resolving, and preventing medication-related issues; establishing individualized treatment goals; and managing, monitoring, and adjusting medication therapies to achieve optimal clinical outcomes. This role also supports quality improvement initiatives, addresses gaps in care, promotes evidence-based practices, and enhances patient satisfaction through patient-centered care and education.

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited college or university.
  • Successful completion of an accredited PGY1 Pharmacy Residency program.
  • Demonstrated clinical experience in either a hospital-based or ambulatory care pharmacy practice setting.
  • Strong knowledge of pharmacotherapy, chronic disease state management, and evidence-based clinical practice guidelines.
  • Ability to assess, monitor, and optimize medication therapy to improve patient outcomes and medication safety.
  • Excellent interpersonal, communication, and patient counseling skills, with the ability to effectively communicate complex clinical information to patients, caregivers, physicians, and other healthcare professionals.
  • Proficiency with electronic medical records (EMR) and clinical documentation systems.
  • Strong critical thinking, problem-solving, and organizational skills with the ability to work effectively within a multidisciplinary healthcare team.

Responsibilities

  • Provide comprehensive medication therapy management and disease state management services for patients in the ambulatory care setting.
  • Manage patients with chronic conditions including, but not limited to, diabetes, hypertension, hyperlipidemia, heart failure, asthma, COPD, anticoagulation therapy, pain management, smoking cessation, and geriatric medication optimization/deprescribing.
  • Utilize evidence-based guidelines and clinical expertise to assess, monitor, and optimize medication regimens to improve patient outcomes and minimize medication-related risks.
  • Establish and monitor individualized treatment goals in collaboration with patients and healthcare providers.
  • Collect, interpret, and evaluate relevant clinical, laboratory, diagnostic, and imaging data to support medication-related decision making.
  • Assess medication therapy for appropriateness, effectiveness, safety, tolerability, adherence, and potential adverse effects.
  • Identify, prevent, and resolve medication-related problems through direct patient care interventions.
  • Recommend and implement clinically appropriate treatment modifications under collaborative practice agreements and organizational protocols.
  • Educate patients and caregivers on medications, disease states, treatment plans, self-management strategies, and adherence techniques to support active participation in care.
  • Provide drug information, educational programs, and clinical in-services to physicians, advanced practice providers, nurses, and other healthcare professionals.
  • Serve as a clinical resource to healthcare teams regarding medication therapy and evidence-based treatment recommendations.
  • Document patient encounters, clinical assessments, care plans, and interventions accurately and timely within the electronic health record.
  • Communicate recommendations, therapy changes, and follow-up plans to referring providers and members of the interdisciplinary care team.
  • Ensure continuity of care through effective coordination and communication across care settings.
  • Precept and mentor pharmacy students, pharmacy residents, interns, and newly hired pharmacists.
  • Support experiential education and residency training programs through clinical instruction and performance feedback.
  • Participate in departmental educational activities and provide presentations as requested.
  • Maintain professional competency through ongoing education and adherence to current clinical practice standards.
  • Assist leadership in the implementation, expansion, and evaluation of ambulatory clinical pharmacy services and programs.
  • Collaborate with physicians, nursing, clinic operations, and administrative leadership to integrate pharmacy services into clinical workflows and enhance patient care delivery.
  • Promote the value and impact of clinical pharmacy services through outcome measurement, patient engagement, and interdisciplinary collaboration.
  • Support quality improvement, population health, and value-based care initiatives designed to improve clinical and operational outcomes.
  • Ensure clinical services are delivered in accordance with departmental policies, professional standards, accreditation requirements, and applicable state and federal regulations.
  • Support appropriate documentation, billing, and reimbursement processes for ambulatory pharmacy services.
  • Identify opportunities to develop, update, and enhance policies, procedures, and clinical protocols.
  • Provide oversight and assume responsibility for the work performed by pharmacy interns, students, and residents, ensuring accuracy and compliance with professional standards.
