Provides ambulatory clinical pharmacy services, including disease management and comprehensive medication review, and Population Health Management under a collaborative practice agreement at multiple sites within the organization. The role involves pharmaceutical care by participating in patient care with medical staff during outpatient clinic visits, designing, implementing, and monitoring patient-specific pharmacotherapy, and promoting safe, evidence-based, and cost-effective drug therapy. It also includes performing Population Health initiatives by reaching out to patients based on clinical criteria to meet provider-delivered care management goals and addressing clinical, medication, or payer needs. The pharmacist will accurately document interventions and outcomes, implement and maintain clinical pharmacist services at multiple ambulatory care sites, and collaborate with other clinicians and pharmacists within the organization. Responsibilities also include assisting in quality improvement processes, providing drug information and education, conducting drug use projects, managing daily workflow, maintaining contact with staff at practice locations, attending provider/staff meetings, preparing for site audits, serving as a preceptor, supporting other offices, and performing special projects as assigned within the scope of licensure.
