An ambulatory care pharmacist will be a vital member of the outpatient focused interdisciplinary team by providing direct patient care under collaborative practice agreements. This position will assist in the development, implementation, and growth of ambulatory care programs within Cameron. The ambulatory pharmacist will support both physician and patients in family medicine, gastroenterology, and specialty pharmacy areas with the opportunity to expand services.

Requirements

  • Completion of an ASHP-accredited pharmacy residency; OR two years of full-time professional level experience as a clinical pharmacist in this setting.
  • Must possess a high level of clinical practice skills, eagerness to teach, and interpersonal skills to communicate effectively with patients, healthcare providers and students.
  • Able to competently use Windows and Microsoft Office Products
  • Bachelor’s degree in pharmacy required: PharmD preferred
  • Active Indiana Pharmacist License or able to obtain license prior to hire date.

Nice To Haves

  • Minimum of 1 year experience preferred but not required.
  • Board Certified Ambulatory Care Pharmacist is encouraged but not required.

Responsibilities

  • Chronic disease state management for diabetes, hypertension, gastrointestinal conditions with opportunities to add more disease states.
  • Given a diagnosis, synthesize a treatment plan, which may include drug therapy and lifestyle modifications, and transition within the continuum of care settings.
  • Identify, resolve, and prevent potential and actual medication-related problems including untreated indications, improper drug selection, subtherapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.
  • Conduct both formal and informal training for nursing staff, medical residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc.
  • Perform drug utilization review, comprehensive medication review, and/or medication reconciliation.
  • Work closely with outpatient pharmacy staff to address any and all cost concerns.
  • Policy development and implementation
  • Developing, monitoring, and reporting quality metrics to Leadership
  • Knowledge of Federal/State laws/regulations regarding delivery of pharmacy services and the scope of practice of a pharmacist.
  • Demonstrated ability to teach and evaluate.
  • Effective verbal/written communication skills at appropriate level.
