About The Position

Emplify Health by Gundersen has an amazing opportunity for an innovative, self-starting, visionary pharmacist to grow as an individual and as part of our pharmacy team! The Ambulatory Pharmacist plays a vital role in providing patient-centered care in our clinic and community-based settings, managing chronic health conditions and optimizing medication therapy.

Requirements

  • Wisconsin Pharmacist License
  • Bachelor’s Degree in Pharmacy; Doctor of Pharmacy (PharmD) preferred.
  • One of the following: 3 years of relevant pharmacy experience (specialty, outpatient/retail, inpatient or outpatient hematology/oncology, or ambulatory care), or Completion of an accredited Pharmacy Residency (PGY1).
  • Strong communication, collaboration, and clinical decision‑making skills.

Nice To Haves

  • Specialized pharmacist certification (e.g., BCACP, BCPS or similar) or active progress toward certification.

Responsibilities

  • Partner with clinicians to ensure seamless care of patients across the continuum
  • Provide clinical support and follow-up assessments for patients
  • Prescribe medications, order labs, authorize refills, and manage pharmacotherapy within established protocols
  • Collaborate closely with physicians, nurses, and clinical staff as well as the rest of the pharmacy team to optimize treatment outcomes.
