The Ambulatory Care Nurse works collaboratively with physical therapists, physicians, and advanced practice providers to provide care to patients primarily in the Raleigh OP Wound Care Center. The nurse uses a collaborative approach to anticipate, integrate, and coordinate patients’ plans of care and maximize their ability to remain in a community setting. In collaboration with physicians, the nurse identifies at-risk patient populations who may require care management services. The nurse serves as a patient and family advocate and as a communication link among physicians, patients, families, the interdisciplinary health care team, and community resources.
