About The Position

The livingwellCARES (LWC) behavioral health specialist reports to the Manager, Care Coordinator and works in collaboration with physicians, Care Coordination staff, internal resources, Patient Navigators, pharmacists, and other healthcare professionals to develop, implement, evaluate and revise the delivery of behavioral health services to the LWC employee/patient population. The LWC behavioral health specialist is a licensed, experience professional who is the clinical expert in the assessment and treatment of adult and families throughout all of our delivery networks and satellite settings in the Yale New Haven Services network. As the clinical expert, the scope of practice includes, but is not limited to, diagnostic assessment, counseling, psychotherapy, brief treatment, behavior health modification, mental health consultation, teaching/mentoring, projects, in-service, clinical practice enhancement, and participation in research. This position will be responsible for collaborating directly with providers, care coordinators, vendors, and community agencies to create a consistent and coordinated delivery of behavioral health services. Actions will be directed toward the goals of prevention, assessment, risk reduction, and health status improvement for high risk patient populations. Activities include, but are not limited to systemic analysis of behavioral health data, care plan development, behavioral health education and advocacy, and collaborating with system and community partners to promote the health of an employee/family population. The LWC behavioral health specialist works from within the context of a patient -centered, team approach, and in continuous partnership with families and physicians to promote: timely access to needed care, comprehension and continuity of care, and the enhancement of patient and family wellbeing. Success will be measured by the results of the process and outcome performance measures of the employee/family EEO/AA/Disability/Veteran

Requirements

  • Master's Degree/Post Master's degree as a behavioral health specialist/social worker from an accredited school for behavioral health/social work
  • 5 years of experience in a clinical setting interacting with individuals and groups to address social determinants of health
  • LCSW and Valid Driver's License
  • Core philosophy or values consistent with a patient-centered approach to care.
  • Culturally effective capabilities demonstrating a sensitivity and responsiveness to varying cultural characteristics and beliefs.
  • Strong interpersonal and leadership skills.
  • Participates effectively as a team member
  • MMR is required.
  • Varivax (chicken pox vaccine) or evidence of prior chicken pox is required.
  • Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids.
  • Tdap strongly encouraged and influenza vaccination required.
  • Must be able to travel to various delivery network locations.
