About The Position

Easterseals PORT Health, a non-profit service provider in North Carolina & Virginia, seeks compassionate, caring, and dedicated individuals to be an AFL Contractor to guide and manage DSPs and appropriate support services for our clients with developmental disabilities or mental health challenges living in their individual homes, support programs or residential settings in Winston-Salem, NC and surrounding areas. Your care will make a huge difference in the lives of the people you will be helping. You will guide your Program Supervisor to provide case management to our clients and their families in collaboration with the MCOs to set personal plans to enable their daily living and social needs based on individual/family desires. You’ll work with the family to coordinate the required services and support needed. The incumbent will provide and promote personal and program development, personnel management, and overall operational administration, including coordination of on-going activities, to meet the established goals and objectives of the program. The incumbent will also develop annual budgets for each assigned cost center and provide for effective monitoring and financial managements of the program. Position Summary: This position is responsible for the supervision and monitoring of Alternative Family Living contractors as well as supervision of developmental disabilities and mental health supports for individuals receiving services within an assigned program. This position is responsible for ensuring best practices, supports based on the personal outcomes of the individual and/or family, compliance with all state, federal, and agency standards as required for assigned services. This position has direct supervision of AFL contractors and Direct Service Providers.

Requirements

  • Bachelor’s degree from an accredited college or university in a human services field
  • QP qualified
  • 2 years of Full-Time work experience with IDD individuals
  • Minimum of Three years of program and supervisory experience required, including experience in residential services
  • Valid driver's license, good driving record and current auto insurance.
  • Must have reliable transportation that will allow movement throughout the program delivery area.
  • Willingness to provide care in various client settings
  • Willing to take a background check

Responsibilities

  • Provide case management to clients and their families in collaboration with MCOs to set personal plans for daily living and social needs.
