Requirements

Current CNA certification or Home Health Aide (HHA) certification is preferred.
Previous experience in home health, long-term care, or a related healthcare setting.
Strong understanding of patient care, safety procedures, and infection control.
Reliable transportation and a valid driver's license to travel to patient homes.
Excellent communication skills and a compassionate, patient-centered demeanor.
Ability to work independently with minimal supervision in a home environment.
CPR/First Aid certification is a plus.

Responsibilities

Provide personal care assistance, including bathing, grooming, dressing, and hygiene support.
Assist clients with mobility, transfers, and ambulation to ensure safety and comfort.
Monitor and document patient health status and report changes to supervising nursing staff.
Support clients with meal preparation, feeding assistance, and nutritional monitoring.
Perform light housekeeping and maintain a clean, safe home environment for patients.
Provide companionship and emotional support to patients and families.
Follow individualized care plans and adhere to home health care protocols.
