The Calvert County Government’s Office on Aging is seeking a Case Manager to assist adults of 60 years of age and older who are homeless or at risk for homelessness, as well as clients in our SOAR (Serving Older Adults with Resources) Program. This person should be able to work independently and contribute as a team player. This position requires the ability to establish and maintain effective working relationships with coworkers, the public, and community partners; communicate effectively, both orally and in writing; understand the psychological, emotional, and social needs of older adults; and deal effectively with the public. The work involves outreach, tracking applicants, case management, and the ability to maintain accurate case notes and records. This position reports to the Long Term Care Manager. Responsible for case management services for the Office on Aging (OOA) Division. The work involves case managing frail, vulnerable elderly, individuals with disabilities and older adults who are homeless in need of home and community-based services. The work requires a degree and experience in the field. The Case Manager reports to the Long Term Care Manager of the Office on Aging.
