About The Position

The Calvert County Government’s Office on Aging is seeking a Case Manager to assist adults of 60 years of age and older who are homeless or at risk for homelessness, as well as clients in our SOAR (Serving Older Adults with Resources) Program. This person should be able to work independently and contribute as a team player. This position requires the ability to establish and maintain effective working relationships with coworkers, the public, and community partners; communicate effectively, both orally and in writing; understand the psychological, emotional, and social needs of older adults; and deal effectively with the public. The work involves outreach, tracking applicants, case management, and the ability to maintain accurate case notes and records. This position reports to the Long Term Care Manager. Responsible for case management services for the Office on Aging (OOA) Division. The work involves case managing frail, vulnerable elderly, individuals with disabilities and older adults who are homeless in need of home and community-based services. The work requires a degree and experience in the field. The Case Manager reports to the Long Term Care Manager of the Office on Aging.

Requirements

  • Bachelor's degree in social work, human services, or related field.
  • Three years of experience providing case management, care coordination or service navigation for older adults.
  • Valid driver’s license.
  • Subject to background investigation.
  • Operation of a County owned vehicle.
  • Requires long periods of standing, frequent walking indoors, repeated bending, crouching, stooping, stretching or reaching; recurring lifting of objects up to 49 pounds; operation of keyboard devices and cell phones.
  • May be required to work evenings and weekends.
  • Worker may be exposed to dirt, dust and potentially hoarding situations during home visits.
  • As court-appointed guardian, worker may be required to identify body upon death.

Responsibilities

  • Oversees and coordinates case management services for the OOA Division.
  • Interviews individuals to assess their medical, social, and financial needs to determine eligibility for programs.
  • Prepares and submits required reports for Federal Older Americans Act, State and local programs.
  • Provides clear and concise documentation on each client.
  • Maintains confidential files on clients.
  • Attends local and state meetings and local outreach events.
  • Attends interdisciplinary meetings as needed to coordinate services.
  • Coordinates services among providers and provides follow-up and assessments as required.
  • Assists clients with necessary applications for services.
  • Assists clients and caregivers with Durable Medical Equipment requests and incontinent supplies.
  • Transports clients to necessary appointments in county vehicle.
  • Participates in all meetings and webinars as required for position.
  • Performs related work as required as directly by supervisor.
