About The Position
Diversified Life Care Partners is seeking experienced healthcare and social services professionals to build a career in Aging Life Care Management. This autonomous, relationship-based role involves assessing needs, solving problems, coordinating care, advocating for clients, and helping families make complicated decisions. An Aging Life Care Manager serves as the central point of coordination for an older adult and their family, looking at the whole person and the whole situation rather than just one episode of care.

- Registered Nurses (RN)

- Speech-Language Pathologists (SLP)
- Strong professional foundation
- Understanding of complex client needs
- Good communication with families and providers
- Ability to think beyond the boundaries of individual discipline
