Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
  • Possession of a valid driver’s license which is not under suspension.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • Satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

Nice To Haves

  • Ability to telework part-time after six months, depending on operational needs.
  • Securely configured high-speed internet connection and work from an approved location inside Pennsylvania (if teleworking).

Responsibilities

  • Gather information from multiple sources to evaluate the level of danger facing the consumer
  • Review facts and determine whether the report of need is substantiated or unsubstantiated
  • Develop plans that address presenting and underlying problems through formal and informal supports
  • Maintain complete and timely records of investigative activities, observations, and contacts in the case system
  • Conduct home visits and phone contacts to review behavior, verify safety, and assess care plan effectiveness
  • Arrange urgent shelter, initiate necessary involuntary procedures, and support court actions when risk cannot be reduced through other measures
