Aging and Disability Support Counselor
About The Position
A full-time Options Counselor position is available with Liberty Healthcare and the Kansas Aging and Disability Recourse Center. The Kansas Aging and Disability Resource Center (ADRC) is a statewide system which supports older individuals and people who are living with disabilities. The ADRC is the “go to” resource for individuals who are seeking information about long term care and other community-based services and supports. Person-centered planning is provided to individuals to support their informed decision making and ability to secure services which will enable them to live as independently as possible.

Responsibilities
- Fielding phone calls and email requests
- Talking with individuals and their families about their needs
- Providing information and answering questions
- Connecting people with appropriate services
- Documenting interactions, services, and relevant information in computer systems
- Occasionally traveling throughout the region to participate in community events and meet with individuals in-person
