About The Position

This role involves providing case management services for inpatients during after-hours shifts. The primary focus is on anticipating and meeting customer needs, ensuring a positive patient experience, and facilitating effective communication between patients, families, physicians, and other healthcare providers. The position requires a strong understanding of patient psychosocial, cultural, ethnic, and religious/spiritual needs, and the ability to develop and implement outcome-based care plans. Responsibilities also include documentation, resource management, and participating in the training of new personnel.

Requirements

  • Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • Current California Licenses
  • Typing 40 words per minute with accuracy.
  • Knowledge of computers, faxes, printers and all other equipment.
  • Knowledgeable in MS Office programs (i.e., Word, Excel, Outlook, Access and Power Point).
  • Ability to deal with responsibility with confidential matters.

  • Knowledge of Healthcare and Managed Care preferred.

Responsibilities

  • Anticipate customer needs and respond appropriately.

  • Function as a liaison between administration, patients, physicians, and other healthcare providers.
  • Interact professionally with patient/family/physicians and involve them in the plan of care.
  • Communicate appropriately and clearly to management, co-workers, and physicians.
  • Identify and address psychosocial needs of patients and family and facilitate consultations with a Social Worker.
  • Formulate a teaching plan based upon identified learning needs and evaluate its effectiveness.
  • Communicate using age-appropriate language.
  • Understand and apply principles of adult learning methods.
  • Receive and review physician’s orders and document completion.
  • Follow through to ensure physician orders are carried out.
  • Consult other departments as appropriate for an interdisciplinary approach.
  • Formulate an outcome-based plan of care, implement and evaluate it.
  • Maintain quality monitors as assigned by Supervisor.
  • Perform other duties as assigned.
  • Participate in orientation, instruction/training of new personnel.
  • Pursue additional education to advance nursing skills and knowledge.
  • Document patient assessment and reassessment, care plans, and other pertinent information completely.
  • Assure complete and proper charging of supplies.
  • Participate in the efficient, effective, and responsible use of resources.
  • Complete daily documentation, required letters, and referrals in a timely manner.
  • Proactively support and practice company mission, vision and values.
  • Follow Employee Handbook guidelines and company policies and procedures.
  • Follow established departmental dress code.
  • Appropriately and accurately maintain time keeping records.
  • Adhere to attendance policy and time off requests.
  • Follow safety policies and procedures.

  • Communicate verbally and in writing through appropriate channels.
  • Maintain confidentiality at all times in compliance with company policy and HIPAA requirements.
  • Complete all required annual Compliance training.
