About The Position

The Aftercare Treatment Specialist reports to the Treatment Coordinator and is an integral part of the treatment team. This role is responsible for meeting all requirements set by the funding source, including reporting and census updates. The position involves conducting risk/needs assessments, developing and implementing service plans for assigned clients, and ensuring compliance with agency policies and funding source standards. Requirements

Bachelor's Degree preferably in a Human Services related field.
Minimum of two (2) years professional experience in mental health counseling.
Experience working with special needs populations.
Practical skills in communication, counseling processes, and resource development.
Working with addictive personalities.
Basic understanding of human behavior.
Utilizing group dynamics to address special needs populations.
Effective written and verbal communication skills.
Ability to act appropriately with clientele.
Basic computer skills.
Demonstrate competency in core curricula facilitation.
Must be able to pass criminal background clearance from funding source.
Must obtain LSI-R Training and Certification within 90 days of hire.
Must be able to facilitate at least one (1) core curriculum program.
Is knowledgeable of and maintains HIPPA standards of privileged information.
Must possess a valid driver’s license and meet agency standards.

Responsibilities

Interact and communicate professionally with clients, staff, and outside contacts.
Maintain client and staff confidentiality.
Attend staff meetings, supervisory conferences, training sessions, and other assigned meetings.
Orient and assess all aftercare clients using approved agency assessment tools.
Complete service plans for assigned clients based on assessment tools and client input.
Monitor and document client progress according to funding source standards and agency policy.
Update service plans with client input.
Schedule and facilitate case management meetings with clients.
Identify and develop resources, and link clients to needed services. Update resource lists quarterly.
Facilitate scheduled workshops, programs, and groups addressing client needs.
Fulfill obligations of training agreements for core curricula programs.
Develop appropriate discharge plans for assigned clients as per contractual agreements.
Facilitate house meetings as needed.
Participate in internal/external Treatment team meetings.
Complete all required reports and documentation.
Demonstrate competency in program-related certifications.
Provide support to treatment assistant functions.
Obtain LSI-R Training and Certification within 90 days of hire.
Attend court hearings and/or parole hearings as subpoenaed or assigned.
Facilitate at least one (1) core curriculum program.
Conduct drug and alcohol screens as directed or needed and report results to the Director.
Perform other duties as assigned.
Provide services to clients once they have fulfilled their criminal justice supervision.
Maintain a valid driver’s license and meet agency standards.
