Visit us online at: https://www.nwths.com/careers Position Summary: The Aftercare Coordinator is responsible for implementing a revised process of meeting patient needs for a continuum of care. The coordinator will meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The coordinator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. The coordinator will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. It is expected that the Aftercare Coordinator will identify implementation barriers and evaluate the new process with the facility leaders using the data. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. In addition, the Aftercare Coordinator will attend to the patient's direct care needs and perform tasks as delegated by the charge nurse or department director. The coordinator will support an organizational culture for Service Excellence and practices Service Excellence standards to all customer groups.

Requirements

Bachelor’s Degree in Mental Health/Behavioral Sciences field preferred
Two years’ experience in Psychiatric/Mental Health field
Must possess a current BLS certification from the American Heart Association and maintain certification throughout employment
Ability to read and write in English
Ability to speak and hear, to allow discussions with employees and peers
Ability to cope with and remain calm under stress
Ability to respond to pages
Ability to evaluate and interpret information and make independent decisions
Bending and lifting (5-10 lbs), grasping, fine hand coordination, pushing and pulling
Ability to understand and interpret financial data

Nice To Haves

A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Responsibilities

Consults with the Social Services, Utilization Management and Outpatient on patient eligibility.
Provides/assists to coordinate transportation for Partial Hospital patients and attends to partial hospital pt. needs as needed.
Maintains current awareness of Outpatient programming.
Maintains Midas data collection process.
Identifies and communicates data driven trends in the step-down process.
Assist in the development of strategies, goals and objectives related to the step-down process.
Consults with facility management, ensuring awareness of step-down trends.
Collaboration with Outpatient Director to evaluate the effectiveness of programming.
Meets the organization’s expectations regarding house-wide requirements.
Attend to the patient's direct care needs and perform tasks as delegated by the charge nurse or department director.
Support an organizational culture for Service Excellence and practices Service Excellence standards to all customer groups.
