Visit us online at: https://www.nwths.com/careers Position Summary: The Aftercare Coordinator is responsible for implementing a revised process of meeting patient needs for a continuum of care. The coordinator will meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The coordinator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. The coordinator will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. It is expected that the Aftercare Coordinator will identify implementation barriers and evaluate the new process with the facility leaders using the data. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. In addition, the Aftercare Coordinator will attend to the patient's direct care needs and perform tasks as delegated by the charge nurse or department director. The coordinator will support an organizational culture for Service Excellence and practices Service Excellence standards to all customer groups.
