AFL in Burke, Caldwell & Catawba Counties
About The Position
The AFL Host provides care and support in an AFL setting, where the individual served lives with the host or the host’s family and blends into the family setting. The host is responsible for the individual 24 hours a day, 365 days a year. Hosts participate in team meetings, carry out the individual’s goals, and submit data to the QP weekly. An AFL Host is not an employee of Skill Creations Inc. The arrangement is governed by an agreement outlining service requirements and financial reimbursement. Compensation consists of a daily rate plus a monthly Room and Board supplement determined by the services provided. The posting identifies the opportunity as Back Up/PRN.
Requirements
- Live in a dwelling that passes Organization, State, and LME-MCO regulations.
- Have a vehicle for transporting the individual with active full insurance, including collision and liability coverage.
- Have clear driving and criminal records.
- Complete RELIAS training provided by Skill Creations Inc. on time.
- Complete CPR/First Aid, Medication Administration, and Behavior Management Programs on time.
