A Path of Care Preferred Pathways in Hugo, OK is seeking a full‑time ADvantage Home Care RN Case Manager. This in‑home care position offers a competitive wage along with excellent benefits. Requirements

• Active RN license in the state of Oklahoma
• No previous ADvantage experience required.
• CPR certification within 60 days of hire
• Valid driver's license and good driving record
• Reliable transportation with proof of insurance
• Ability to travel long distances and adapt to frequent schedule changes
• Prioritize high‑quality health care
• Friendly and positive attitude
• Work well in fast‑paced environments

Responsibilities

• Arranging and coordinating service plans
• Providing nursing assessments and education
• Collaborating with case managers and other care team members
• Ensuring documentation meets agency and ADvantage program standards
• Supporting members and families with professionalism, compassion, and respect
• Maintaining regular attendance and punctuality
• Home visits with members
