About The Position

A Path of Care Preferred Pathways of Hugo, OK is hiring a full-time ADvantage Case Manager serving Lawton, OK and the surrounding area. The Case Manager provides in-home case management to help members remain safe, independent, and supported in their homes. The role includes assessing members’ needs, developing plans of care, monitoring members monthly, initiating appropriate preventive interventions, and coordinating services. The Case Manager also participates in in-service programs and ADvantage training.

The position uses flexible self-scheduling. Work is split between remote work and travel to complete in-home member assessments and monitoring. Requirements

• Oklahoma RN or LPN license.
• Bachelor’s degree.
• One year of paid professional experience working with an aging or disabled population.
• CPR certification within 60 days of hire for RN/LPNs.
• Valid driver’s license and good driving record.
• Reliable transportation and proof of insurance.
• Ability to travel long distances.
