About The Position

Vecinos is a non-profit, 501(c)3 Free Clinic that provides bilingual integrated primary and mental health care, mobile clinic services, health education, and case management to uninsured, low-income adults in the far-western NC counties. We offer a positive, supportive, and team-oriented atmosphere. We work in a non-traditional environment with limited resources. Schedules and hours vary throughout the year. The Advanced Practitioner works under the supervision of the Medical Director and provides primary medical care with a strong focus on preventive care and integrated wellness in collaboration with the mental health team.

Requirements

  • Licensed/certified Nurse Practitioner or Physician Assistant in the state of North Carolina
  • Advanced English and Spanish fluency with excellent written and oral communication skills
  • Willing to participate in outreach events in surrounding counties, as needed
  • Cultural sensitivity, a caring and confident demeanor, flexibility, dependability, professional attitude and desire to serve uninsured/underserved populations

Nice To Haves

  • Minimum 2 years’ experience as an advanced practitioner
  • At least 2 years’ experience in a multidisciplinary care team setting
  • Native or near-native fluency
  • Working knowledge of farmworker-specific medicine

Responsibilities

  • Perform primary care medical evaluations, diagnosis and treatment of acute and chronic diseases with appropriate follow-up, diagnostic tests, medications, referrals, documentation, and case management
  • Perform procedures as needed (dependent on previous experience)
  • Complete appropriate documentation and communication within the EHR
  • Provide evidence based patient care across outpatient, telehealth, and mobile clinic sites throughout Western North Carolina.
  • Develop and oversee implementation of patient care plans
  • Participate in monthly Quality Improvement meetings with the Medical Director
  • Assist all staff in reaching clinical measures and quality goals
  • Active participation in weekly team meetings and monthly team plannings, providing insight into programmatic design and initiatives
  • Precept and supervise volunteers and university students, as needed
  • Work collaboratively with the clinic team, other medical providers, mental health therapists, and registered nurses to provide integrated healthcare to patients
  • Collaboration with the Clinical Director and Outreach Director to provide ongoing clinical support and outreach medical services
