About The Position

Whole Family Health Center is seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to join our medical team. In this role, you will provide comprehensive, patient-centered care while working collaboratively with physicians, nursing staff, and other members of the healthcare team. Requirements

  • Graduate of an accredited school of nursing.
  • Graduate of an accredited Nurse Practitioner program.
  • Current Advanced Practice Registered Nurse (APRN) license.
  • Current Nurse Practitioner certification in the applicable specialty area, if required.
  • Current Basic Life Support (BLS) provider certification.
  • Eligible for hospital privileges.
  • Level 2 background clearance required.

Nice To Haves

  • Three (3) or more years of Nurse Practitioner experience preferred.
  • Spanish and/or Creole language skills are a plus.

Responsibilities

  • Perform comprehensive, age-appropriate histories and physical examinations for patients with acute, chronic, and complex healthcare needs.
  • Assess patient health status, establish differential diagnoses, and develop individualized plans of care.
  • Order and interpret appropriate laboratory, diagnostic, therapeutic, and radiographic studies.
  • Prescribe appropriate pharmacologic and non-pharmacologic treatments.
  • Implement and monitor interventions to help patients regain or maintain optimal health.
  • Evaluate patient progress and modify treatment plans as appropriate.
  • Educate patients and families and support informed healthcare decision-making.
  • Facilitate patient transitions within and between healthcare settings when necessary.
  • Accurately and promptly document assessments, progress notes, treatment plans, and other required information in the Electronic Health Record (EHR).
  • Provide clinical instruction and guidance to nursing staff as appropriate.
  • Maintain current knowledge of clinical practices, healthcare processes, and applicable standards of care.
  • Provide coverage at other clinic locations occasionally, as needed.

  • Annual salary of $142,000 – $152,000
