About The Position

The Advanced Practice Provider provides comprehensive patient care within a multidisciplinary clinical environment, utilizing advanced clinical knowledge, evidence-based practices, and independent clinical judgment within the scope of licensure, credentialing, and established practice standards. The role is responsible for the assessment and management of patients, the development and implementation of plans of care, and the performance of image-guided and other clinical procedures for which the provider is appropriately trained, credentialed, and privileged. The Advanced Practice Provider works collaboratively with physicians and other members of the interdisciplinary care team to support the delivery and coordination of high quality patient care across inpatient, outpatient, and procedural settings. Responsibilities may include image guided procedures, inpatient rounding, outpatient clinical care, patient and family education, care coordination, discharge planning, consultation, quality improvement, staff education, and participation in research and programmatic initiatives.

Requirements

College or university degree from an accredited school/college.
Knowledge and skills developed through a combination of education and a minimum of 2 years of relevant work experience in a related job discipline.
Current and/or eligible for State of Illinois licensure/registration required.
Eligibility to be privileged and credentialed through the Medical Staff Office (MSO).

Advanced degree (or equivalent, if license received prior to July 1, 2001) from an accredited school/college of nursing or completion of an accredited Physician Assistant educational program accredited by the Accreditation Review Commission on Education for the Physician Assistant, Inc. (ARC-PA).
Experience working in a procedural, surgical, interventional, or other related clinical environment.
Experience performing or assisting with clinical procedures.
Experience providing care in an interdisciplinary health care environment.
Current Basic Life Support certification preferred or required based upon departmental and clinical responsibilities.
Demonstrated clinical judgment, critical thinking, and problem-solving skills necessary to independently assess patients and respond appropriately to changes in clinical condition.
Knowledge of clinical assessment, diagnosis, treatment planning, and patient management within the applicable scope of practice.
Ability to obtain and evaluate comprehensive medical histories and other relevant clinical information.
Ability to perform clinical and image-guided procedures safely and effectively following appropriate training, credentialing, and privileging.
Strong verbal and written communication skills, including the ability to communicate complex clinical information effectively with patients, families, physicians, and other members of the health care team.
Demonstrated ability to provide effective patient and family education.
Strong interpersonal and collaborative skills with the ability to function effectively as a member of an interdisciplinary care team.

Responsibilities

Provides comprehensive clinical assessment and management of patients within the scope of practice, including obtaining patient histories, performing physical examinations, developing differential diagnoses, and establishing appropriate plans of care.
Performs image-guided and other clinical procedures for which the provider has been appropriately trained, credentialed, and privileged, independently and/or in collaboration with supervising or attending physicians as appropriate.
Provides care to patients across inpatient, outpatient, and procedural settings, including participation in inpatient rounding and outpatient clinical activities.
Orders, reviews, and interprets laboratory studies, diagnostic imaging, and other diagnostic tests within the provider's scope of practice and uses findings to inform clinical decision making and treatment planning.
Develops, implements, and monitors individualized plans of care and modifies treatment plans based on patient response, clinical findings, and established standards of care.
Provides patient and family education regarding diagnoses, procedures, treatment options, anticipated outcomes, follow up care, and other information necessary to support informed health care decisions.
Coordinates patient care across the continuum, including consultation, referrals, discharge planning, follow up care, and communication with members of the interdisciplinary care team.
Collaborates and consults with physicians, nurses, Advanced Practice Providers, and other health care professionals to promote safe, effective, and coordinated patient care.
Documents patient assessments, clinical findings, procedures, treatment plans, and other required information accurately and timely within the Electronic Medical Record and other applicable clinical systems.
Identifies changes in patient condition, evaluates treatment effectiveness, and initiates appropriate clinical interventions or escalation of care when necessary.
Participates in the development and implementation of clinical protocols, workflows, and other initiatives designed to support consistent and effective patient care.
Participates in quality improvement and performance measurement activities, including evaluating clinical outcomes, patient safety, quality of care, and opportunities for process improvement.
Supports patient care program development and other departmental initiatives aimed at improving clinical operations, patient access, quality, and outcomes.
Participates in educational activities for patients, families, staff, trainees, and other members of the health care team as appropriate.
Participates in research, evidence-based practice, and other scholarly or programmatic activities as appropriate to the department's needs.
Designs patient education programs that include information required to make informed health care and treatment decisions.
Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
Performs other related work as needed.
