About The Position

The APP role involves providing advanced healthcare services, diagnosing illnesses, prescribing medications, and collaborating with interdisciplinary teams to deliver holistic patient-centered care. With expertise in primary and specialty care, Nurse Practitioners play a vital role in improving health outcomes and enhancing the quality of life for patients. This is a value-based care model position with over 20 clinic locations in South Florida, operating in a 100% outpatient primary care clinic setting seeing patients 18+. The role includes a dedicated Medical Assistant and additional clinic support team, with a Monday – Friday schedule (some clinics have extended hours) and telehealth availability for patients. Requirements

  • Current Nurse Practitioner and Registered Nurse/ or Physician Assistant licensure in the State of Florida.
  • National board certification as a Nurse Practitioner or Physician Assistant from an accredited certifying agency is required.
  • Master’s degree in Nursing (for Nurse Practitioners) or completion of an accredited Physician Assistant program (for Physician Assistants) is required.
  • Two or more years of clinical experience as a Nurse Practitioner or Physician Assistant in a physician practice, clinic, or hospital setting is required.
  • Bilingual in English and Spanish required.

Nice To Haves

  • Board certification in the applicable specialty as a Nurse Practitioner or Physician Assistant is preferred.
  • Doctor of Nursing Practice (DNP) preferred for Nurse Practitioner candidates.
  • Communicates effectively, speaking multiple languages is preferred.

Responsibilities

  • Maintains confidentiality of all patient records.
  • Checks in with clients about test results and prognoses.
  • Advises patients regarding prescriptions and treatment plans.
  • Follows all federal, state, and nursing guidelines, regulations, and standards.
  • Orders and interprets diagnostic tests, including x-rays, blood work, and scans.
  • Refers patients to specialists if necessary and analyzes medical records and reports.
  • Updates patient charts and uploads to Electronic Health Record (EHR) software.
  • Works with a team of nurse practitioners, registered nurses, and doctors to provide quality patient care.
  • Examines patients, including discussing medical history and symptoms and performing physicals.
