About The Position

HealthPartners is seeking a patient-focused Advanced Practice Provider to join their Digestive Care/Gastroenterology practice at the Specialty Center on Phalen Blvd (St. Paul). This role involves managing a full patient caseload, providing new patient assessments for liver and general GI diseases, ongoing chronic disease management, and supporting patients between visits. The Advanced Practice Provider will be an integral part of the GI team, contributing foundational and essential patient care to the HealthPartners patient population with GI and liver disease. Requirements

  • Successful completion of an approved Nurse Practitioner program or accredited Physician Assistant program.
  • Licensed as a Physician Assistant OR currently licensed as an APRN in the state of MN and current MN RN license.
  • Current APP certification as Physician’s Assistant OR current APRN Specialty Certification.
  • For PA, must be approved by Minnesota Board of Medical Practice for prescription writing privileges.
  • Current BLS.
  • Current DEA certification.
  • Must maintain CME requirements.
  • 1-year experience as a NP or PA.

Responsibilities

  • Obtains history and examines patient, including reviewing patient's records, discussing patient problems, performing examinations, and documenting in the patient record.
  • Orders appropriate labs and follow-up care, documenting orders in the patient record and notifying appropriate personnel.
  • Discusses with and instructs patient and family members regarding treatment and follow-up, documenting in the patient record and providing written instructions.
  • Communicates and follows up with patients in between clinic visits, reviewing lab and test results, making recommendations for treatment next steps and modifications, and potentially conducting Scheduled Telephone Visits.
  • Interacts with other healthcare professionals by obtaining pertinent patient information, obtaining consults, and providing feedback or instructions.
  • Completes all required paperwork in a timely fashion, maintaining and reviewing patient records, charts, and other pertinent information, and posting tests and examination results.
  • Collaborates and coordinates with other health professionals, making and accepting referrals, consulting with, cooperating with, and referring to all other types of healthcare providers. Discriminates between situations requiring independent management, collaborative management with a physician, or further referral. Collaborates with physicians when unusual incidents or changes in patient condition occur. Coordinates care involving other healthcare providers. Functions as a mentor/resource and educator for other healthcare team and staff.
  • Coordinates the flow of Inpatient Service by reviewing daily patient lists, prioritizing consultations and rounding schedules, developing and reviewing work plans with rounding physician partners, communicating with partner Inpatient APP to coordinate consult services and balance workloads, and assisting in the assignment of residents and students as necessary.
  • Discusses and develops diagnosis and treatment plan with staff gastroenterologist, assessing and completing H&P on new consults, ordering diagnostic testing, evaluating treatment for assigned patients, communicating information to physician partner, and designing follow-up plans for patients seen in the clinic upon hospital discharge. Documents assessment, treatment, evaluation, and follow-up plan in the electronic medical record.
  • Discusses with and instructs patient/family members regarding treatment and follow-up, interacting with patients and family members to involve them in the plan of care, educating patient/family members regarding diagnosis and treatment and documenting this in the electronic medical record, and prescribing medications as indicated.
