About The Position

Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. Accountable for attaining budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. The position also requires meeting Advanced Practice Provider professional focus and goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, maintenance, illness prevention, and applying a holistic healthcare model. Management of chronic diseases and acute illness with broad knowledge in procedural skills, coordination with specialists, and utilization of community resources is essential. The role advocates for access to healthcare with a focus on structured evaluation, early intervention, and health promotion, consulting with supervising physicians or team members as needed, and promoting the Patient-Centered Health Home model of care. Excellent customer service and care are paramount, requiring courteous interaction with patients, families, and staff, awareness of wait times, and providing appropriate educational materials. The provider is accountable for patient status as measured by clinical quality measures, promoting daily huddles, incorporating ancillary health team members, setting and monitoring self-management plans, assuring clinical procedures, and maintaining up-to-date clinical protocols. Chart and peer review responsibilities must be completed in a timely manner. The role also assists the patient care team with performance improvement, staff education, and FHCN promotion, potentially participating in the rotation of medical, physician assistant students, and dental residents. Participation in Network Health Fairs and community events, as well as meaningful participation in site and provider meetings, is expected. The provider must meet Network and credentialing expectations for licensure and employment and perform other duties as assigned.

Requirements

Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including an understanding of the application of the theory and practices of the profession to the operation of the organization as part of a master’s degree (or other applicable extended training program).
Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
Licensed in the state of California as a Nurse Practitioner and/or licensed in the state of California as a Physician Assistant.
Requires employees to effectively communicate their professional opinions and extrapolations of information they collect, synthesize/analyze.
Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts, and other visual forms.
Duties require the preparation and execution of presentations to large groups.

Responsibilities

Provide comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
Manage chronic diseases and acute illness with a broad range of knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
Advocate access to health care with a focus on structured evaluation, early intervention, and health promotion.
Consult with supervising physician or other team members on cases that need further input.
Works within and helps to promote the Patient-Centered Health Home model of care.
Provide excellent customer service and care.
Consistently behave courteously when interacting with patients/ family members, support staff, and Network staff.
Remain aware of wait time during the daily workflow.
Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
Accountable for patient status as measured by relevant clinical quality measures.
Promote and participate in daily huddles.
Incorporate the participation of ancillary health team members in managing the care of patients/families.
Set and monitor self-management plans.
Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
Complete chart and peer review responsibility in a timely manner.
Assist the patient care team with performance improvement efforts, staff education, and FHCN promotion.
Assist the patient care team with performance evaluations and quality improvement.
Where relevant participate in rotation of medical, physician assistant students and dental residents.
Participate in Network Health Fairs and other community events.
Participate in site and provider meetings in a meaningful way.
Meet Network and credentialing expectations for licensure and employment.
Perform other duties as assigned.
