Advanced Practice Provider - Palliative Care
About The Position
Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for an Inpatient and Outpatient position to join our team at Franciscan Health Michigan City to meet the growing demands for Hospice and Palliative Care services. The ideal candidate will have past Palliative or Home Health Care experience, with a focus on clinical excellence, and patient satisfaction. The candidate will display exceptional interpersonal skills and a desire to develop strong ties with the community and existing medical staff. The provider will need to work collaboratively with a physician to coordinate and deliver the highest quality of patient care.
Requirements
- Master's Degree
- Physician Assistant Certified (PA-C) or Nurse Practitioner (NP) or Nationally Recognized Certification
- Basic Life Support Program (BLS) - American Heart Association
- Palliative Care experience
- Eligibility to become certified within 2 years is required
Nice To Haves
- Past Palliative or Home Health Care experience
- Certification as a Hospice and Palliative Nurse (ACHPN®) is preferred
Responsibilities
- Coordinate and deliver the highest quality of patient care in collaboration with a physician.
- Provide Inpatient follow-ups and new consultations.
- Clarify care planning with patient’s family.
- Offer Telehealth services.
