About The Position

Community Health Network is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join their Palliative Care team in Indianapolis, IN. This is a 1.0 FTE position, working weekdays. The provider will work autonomously and collaboratively as a "float" provider across Community Hospitals North, East, South, and Community Heart & Vascular Hospital in Indianapolis. The role focuses on delivering palliative and health care services, including comfort measures, end-of-life care, symptom management, and assisting with consults and new admissions. The Palliative Care program is described as the most comprehensive of its kind in Indiana, committed to helping patients and families understand their medical journey and providing physical and emotional comfort.

Requirements

Possess 1-3 years’ Experience as a licensed Nurse Practitioner in Palliative Care
Be comfortable assessing patients independently
Be compassionate caregivers with excellent organizational, communication, and critical thinking skills
Possess highly developed interpersonal and group dynamic skills, as well as proven skills as a consultant/educator to friends, families, and loved ones
Educational background consistent with requirements for licensure and certification within the state of Indiana
Licensure and certification within the state of Indiana
Flexibility to travel between hospitals in Indianapolis

Responsibilities

Work autonomously and collaboratively with our palliative care multi-disciplinary teams as a "float" provider at our Indianapolis hospitals to deliver palliative and health care services to patients
Focus on the comfort measures of patients, end of life care, symptom management, as well as assisting with consults and new admissions
