About The Position

Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Indianapolis, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need. This is a 1.0 FTE position, weekdays. This provider will work autonomously and collaboratively with our palliative care multi-disciplinary teams as a "float" provider at our Indianapolis hospitals (Community Hospitals North, East, South, and Community Heart & Vascular Hospital) to deliver palliative and health care services to patients. The role focuses on comfort measures, end-of-life care, symptom management, and assisting with consults and new admissions. Flexibility to travel between hospitals in Indianapolis is required. Requirements

1-3 years’ Experience as a licensed Nurse Practitioner in Palliative Care
Comfortable assessing patients independently
Compassionate caregivers with excellent organizational, communication, and critical thinking skills
Highly developed interpersonal and group dynamic skills
Proven skills as a consultant/educator to friends, families, and loved ones
Educational background consistent with requirements for licensure and certification within the state of Indiana

Responsibilities

Deliver palliative and health care services to patients.
Work autonomously and collaboratively with palliative care multi-disciplinary teams.
Focus on comfort measures of patients, end of life care, and symptom management.
Assist with consults and new admissions.
