About The Position

The Medical College of Wisconsin’s Department of Medicine, Division of General Internal Medicine, is seeking a Nurse Practitioner or Physician Assistant to join the Primary Care Outpatient Services at Froedtert Hospital Specialty Clinics in Milwaukee, Wisconsin. The APP will function as a member of a multidisciplinary team and contribute to excellence in patient care, safety, research, quality improvement, teaching, and program administration. Our Advanced Practice Providers (APPs) work closely with physicians to deliver coordinated, team-based care! Join a group that provides comprehensive outpatient services grounded in scientific knowledge, clinical expertise, and compassionate care for adults—including those with multiple chronic medical conditions. Our providers help patients understand disease prevention and manage conditions such as cardiovascular disease, endocrine disorders, cancer, and musculoskeletal disease. We also support patients with chronic illnesses by coordinating care with specialty providers. Through collaboration with nursing staff, social workers, and community health workers, we deliver comprehensive and coordinated care. The ideal candidate is an experienced Primary Care APP who seeks to work in a collaborative academic environment. The APP should have a strong interest in providing primary care for complex adult patients with multiple chronic diseases, including cancer or obesity and in supporting patients through transitions of care from the inpatient to the outpatient setting. Successful candidates will have the opportunity to participate in the creation of new, team-based care programs in three primary areas including: Women's Health, Obesity Management, and Cancer Survivorship, including the care of adults currently living with cancer, who are experiencing therapy related toxicities like CHF, DM and HTN.

Requirements

  • Advanced Practice Nurse: Master’s Degree or higher in Nursing from a nationally accredited organization.
  • Physician Assistant: Bachelor’s Degree or higher in Physician Assistant Studies from an ARC-PA-accredited organization.
  • Wisconsin RN License
  • Wisconsin Advanced Practice Nurse Prescriber License
  • American Heart Association Advanced Cardiac Life Support (ACLS)

Nice To Haves

  • 2+ years of APP Primary Care experience preferred
  • 5+ years of APP experience is a plus!
  • Prior experience in cancer care, survivorship, or oncology primary care (as APP or RN).
  • Familiarity with hereditary cancer syndromes and cancer after-care programs.
  • Ability to collaborate with oncology teams and manage complex cancer survivor populations.

Responsibilities

  • Collaborate with an interdisciplinary team in diagnosing and treating conditions requiring general internal medicine intervention in an outpatient setting.
  • Manage primary care needs, acute concerns, and co-morbid conditions such as diabetes, hypertension, heart failure, asthma, and anemia.
  • Write accurate and timely progress notes, procedure notes, and clinical documentation.
  • Facilitate communication with patients, families, and staff to promote continuity of care.
  • Use evidence-based knowledge to guide clinical decision-making for wellness, women’s health, and acute and chronic conditions.
  • Conduct post-hospital and emergency department follow-up visits.
  • Perform comprehensive histories and physical exams, identifying normal and abnormal findings across physical, cognitive, and psychosocial domains.
  • Order, perform, and interpret diagnostic tests, and communicate results appropriately.
  • Develop and monitor pharmaceutical and non-pharmaceutical treatment plans.
  • Educate patients on disease processes and treatment options.
  • Perform procedures as trained and credentialed.
  • Document care in the Electronic Medical Record (EMR) and complete required records.
  • Collaborate and consult with physicians and other healthcare providers as needed.
  • Counsel patients and families on health maintenance and preventive care.
  • Attend required training and meet institutional and regulatory requirements, including CMS and Joint Commission standards.
  • Stay current with developments and trends in the Nurse Practitioner or Physician Assistant profession through education and professional engagement.
