About The Position

Penobscot Community Health Care is seeking an experienced BC/BE Nurse Practitioner or Physician Associate interested in working with the adult and geriatric population as part of our Community Care and Geriatrics (CCG) team. You will provide in-home care as well as direct care and geriatric services in local nursing homes and other long-term care settings in Greater Bangor. CCG is a well-established team of highly skilled physicians, advanced practice providers, licensed clinical social workers, and nurses focused on finding the right health care solutions for patients, caregivers, and their families.

Requirements

Two years’ experience in clinical practice as a Nurse Practitioner or Physician Associate.
Background in Community Health, Family Practice or Adult-Gerontology is preferred.
Having a valid driver’s license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required.
Any concerns raised by the results of a driving record check will be subject to an individualized review to determine whether the infraction is relevant to the individual’s ability to perform the responsibilities of the position, in accordance with PCHC’s Automobile Safety and Background Check policy.

Responsibilities

Provide in-home care.
Provide direct care and geriatric services in local nursing homes and other long-term care settings.
Share a very reasonable rotating call for PCHC CCG patients which include patients seen in nursing homes, boarding homes, assisted living and homebound patients.
Travel and work in patient homes and in PCHC/CCG facilities.
