About The Position

Franciscan Physician Network is seeking a full-time Nephrology Advanced Practice Provider to join our team at Franciscan Health Hypertension & Nephrology located in Lafayette, Indiana. The Nurse Practitioner or Physician Assistant will support the Nephrology department by seeing patients in the clinic and in the outpatient dialysis units. The APP will rotate one to two days per week at these sites with one physician and one APP.

Requirements

• Master's Degree - Required
• Physician Assistant Certified (PA-C) – Required - or Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association - Required
• Past or present nephrology experience is ideal.
• Strong focus on clinical excellence and patient satisfaction.
• Exceptional interpersonal skills.
• Desire to develop strong ties with the community and existing medical staff.

Nice To Haves

• Experience in Nephrology is preferred.

Responsibilities

• Seeing patients in the clinic and in the outpatient dialysis units.
• Rotating one to two days per week at outreach locations (Crawfordsville, Monticello, Rensselaer).
• Working collaboratively with a physician to coordinate and deliver the highest quality of patient care.
• Supporting the Nephrology department.
