About The Position

Franciscan Physician Network is seeking a full-time Primary Care and Geriatric Medicine Nurse Practitioner or Physician Assistant to meet the growing demand for chronic disease services at our Franciscan Healthy Living Center in Lafayette, Indiana. The Provider will work in our Transition Clinic focusing on chronic disease management and will be the sole provider onsite during shifts. The Advanced Practice Provider will lead a team-based care model that consists of respiratory therapists, dieticians, clinical nurses, and social workers. The Healthy Living Center provides services that focus on chronic disease, diabetes, and weight loss.

Requirements

• At least two years’ experience in primary or urgent care
• Strong focus on clinical excellence, and patient satisfaction
• Exceptional interpersonal skills
• Desire to develop strong ties with the community and existing medical staff
• Ability to work collaboratively with the care team to coordinate and deliver the highest quality of patient care
• Team focused with excellent leadership qualities
• Master's Degree
• Physician Assistant Certified (PA-C) or Nurse Practitioner (NP) or Nationally Recognized Certification
• Basic Life Support Program (BLS) - American Heart Association

Responsibilities

• Physical assessment
• Ordering medications
• Following guideline therapies
• Helping transition from hospital to home
• Educating on disease process
• Outreach to patients
• Coordinating care with primary care physicians, specialists, and other programs like home care and paramedicine
• Potential outreach to satellite office located in Crawfordsville, IN
