About The Position
St. Mary Physicians Group, a member of Trinity Health, Mid-Atlantic, is looking for an Advanced Practice Provider to join our Cardiology Langhorne team! This is an established practice with an opportunity to build a robust patient panel. The role offers organizational support to drive practice growth and community engagement, utilizes an integrated Epic EMR, and benefits from a strong referral network within Trinity Health Mid-Atlantic.
Requirements
- Current and valid license to practice medicine in the State of Pennsylvania.
- Possess current, unrestricted federal and Pennsylvania permits to prescribe medications, including controlled substances.
- Hold a degree as a Physician Assistant or Nurse Practitioner from an accredited institution
Responsibilities
- Provide comprehensive care for patients across the lifespan
- Diagnose acute and chronic conditions
- Emphasize preventive care and population health management
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Maintain timely and accurate documentation
- Participate in quality and performance improvement initiatives
