About The Position

Independent, physician-owned primary care practice seeking an Advanced Practice Provider (APP) to provide preventive care, chronic disease management, and continuity-focused patient care.

Requirements

• Graduate of accredited PA or NP program.
• Current NYS license as PA (physician's assistant) or NP (nurse practitioner) certified by the NYS Education Department.
• Experience in health care delivery, preferably in an outpatient clinic setting and in family medicine or primary care preferred.

Responsibilities

• Perform comprehensive evaluations and follow-up visits
• Manage chronic conditions (HTN, DM, COPD, CKD, etc.)
• Provide preventive and wellness care
• Order/interpret labs and diagnostics
• Coordinate care with specialists and team members
• Provide patient education and counseling
• Document in EMR/EHR
