About The Position

This role involves providing medical management for patients referred to the practice. The APP will conduct comprehensive patient assessments, including reviewing medical history, family history, social history, and current symptoms. Responsibilities include ordering and interpreting diagnostic tests, managing medications, referring patients to other providers, and counseling patients on their care. The APP will also follow patients within the hospital as a consultant, documenting care, placing orders, and ordering appropriate testing. This is a part-time position.

Requirements

  • Graduate of a nationally accredited and/or state approved PAC educational program approved by the American Medical Association.
  • Pennsylvania Certification as a PAC.
  • Credentialing - Competencies in accordance with current credentialing and/or competency requirements.
  • Current BLS Healthcare Provider card.

Responsibilities

  • Establishes preliminary diagnoses of physical abnormalities under the direction of the supervising physician.
  • Recommends further testing and examinations to appropriate cooperating services.
  • Provides preventative services such as weight loss/diet modification and smoking cessation counseling.
  • Maintains and documents complete medical records and completes patient reports.
  • Participates in the development of written standing orders and/or protocols.
  • Completes prescription refills per Collaborative Agreement with supervising physician.
  • Reviews daily lab and hospital reports with supervising physician and addresses them as appropriate.
  • Accepts other duties and special assignments as designated by the Medical Director.
  • Sees patients in the office referred for medical management, including EKG, reviewing patient history, symptoms, and performing assessments.
  • Schedules patients for further testing, including echocardiograms, stress tests, radiologic studies, and laboratory studies.
  • Provides medication management and referrals to other providers as necessary.
  • Reviews test results with the patient, providing counseling and coordination of care.
  • Follows patients within the hospital as consulted, documenting care, placing orders, and ordering testing as appropriate.
