About The Position

The Advanced Practice Provider (APP) will provide care within the Department of Family Medicine, serving the Whole Person Pain Service (WPPS) as a 0.25 cFTE. The WPPS will provide holistic chronic pain care for primary care patients established in the department of family medicine. The service will be 100% virtual and will serve as the chronic pain hub for patients in the department who are currently prescribed chronic opioid medications. The APP in this role will be responsible for two main functions: chronic pain patient care and care coordination. The total FTE commitment is 0.25 cFTE, which includes 8 hours of appointable clinic hours per week and 2 hours per week of clinical administrative time (paperwork, documentation, required meetings, required trainings, etc.).

Requirements

  • Masters/Doctor of Nursing practice degree or graduation from an NCCPA-approved physician assistant program.
  • Licensed as a Registered Nurse and Nurse Practitioner or Physician Assistant in the State of Colorado.
  • Completion of required 1,000 hours (provisional prescription authority/mentorship) or other minimum as defined by State of Colorado in order to obtain full prescriptive authority and DEA registration.
  • Medicare/Medicaid credentials.
  • Unrestricted DEA license and current CPR license.
  • National certification by ANCC or AANP as family or adult nurse practitioner (if FNP).

Nice To Haves

  • Minimum 3+ years (full time) of FNP/PA experience working in a Patient Centered Medical Home/Ambulatory Family Medicine practice
  • Experience working with Epic or another EHR.
  • Knowledge and skills necessary to provide care appropriate to the age of the patients served; the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs, and to provide the care needed as described in the office’s policies and procedures.
  • Demonstrates cultural humility and commitment to minimizing implicit bias in clinical care
  • Ability to consult with supervisory physicians on patient care and administrative issues.
  • Able to document history, physical examinations, assessments and treatment plans for each patient by using the electronic medical record and enter follow-up diagnostic testing and laboratory studies that have been ordered.
  • Able to discuss and promote wellness for all patients.
  • Knows when and how to refer patients to other health care professionals when the care that needs to be provided is beyond their scope of practice.
  • Proven effectiveness in working with individuals from diverse ethnic, cultural, and socio-economic backgrounds
  • Working knowledge of HIPAA.
  • Ability to prioritize and respond to emergencies in a calm manner.
  • Ability to verbalize information/directions clearly and concisely.
  • Ability to administer patient medications properly.
  • Ability to maintain confidentiality of sensitive information.

Responsibilities

  • Provide patient care to patients with chronic pain in a virtual setting.
  • Perform intake, routine follow-up, and group visits for patients on chronic opioids.
  • Participate as needed in chart review and clinical coding compliance activities.
  • Collaborate as appropriate with medical staff in the care of complex patients.
  • Interact with other health professionals to create guidelines and coordinate patient care.
  • Triage the WPPS inbasket and telephone line and support patients as needed.
  • Using opioid registries, proactively outreach patients and/or PCPs who may benefit from the WPPS.
  • Using opioid registries determine the highest risk patients in need to be discussed at collaborative care conferences.
  • Maintain eligibility for School of Medicine Faculty.
  • Keep abreast of the latest developments in field, both academically and through applied skills.
  • Attend and present at conferences, meetings and trainings to maintain certification and enhance professional skills; participates in faculty development activities.
  • Accept other responsibilities assigned by the Medical Director.
  • Develops, implements, and evaluates the strategy for quality improvement.
