About The Position

Advanced Practice Provider (APP) – Family Medicine Carilion Family Medicine – Colonial Avenue | Roanoke, Virginia Carilion Clinic is seeking a dedicated Advanced Practice Provider (APP) to join the team at Carilion Family Medicine – Colonial Avenue. This is an excellent opportunity for a Nurse Practitioner (NP) or Physician Assistant (PA) who enjoys building long-term relationships with patients and providing high-quality, comprehensive primary care. As an APP at Carilion Family Medicine – Colonial Avenue, you will work collaboratively with physicians, APPs, nurses, and other members of the care team to provide patient-centered care across the lifespan. The role offers the opportunity to practice with a high degree of autonomy while benefiting from the resources and support of Carilion Clinic's integrated health system.

Requirements

  • Strong clinical assessment, communication, and interpersonal skills.
  • Commitment to collaborative, team-based, patient-centered care.
  • Graduate of an accredited School of Nursing and Nurse Practitioner program or an accredited Physician Assistant program required.
  • 1 year of clinical experience preferred.
  • Licensed or eligible for licensure as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) in the Commonwealth of Virginia.
  • Nurse Practitioners must also hold a current Registered Nurse (RN) license in Virginia.
  • Must be eligible for prescriptive authority in accordance with state regulations and as authorized by the Virginia Boards of Nursing and Medicine.
  • Current Basic Life Support (BLS) certification required.
  • Pediatric Advanced Life Support (PALS) or Neonatal Resuscitation Program (NRP), and Advanced Cardiac Life Support (ACLS) certifications required as appropriate to the position.
  • Must maintain all credentialing and licensing requirements of Carilion Clinic, the Virginia Board of Medicine or Nursing, and applicable certifying bodies.
  • Ability to work collaboratively with physicians and other healthcare providers.
  • Strong written and verbal communication skills required.
  • Proficiency with electronic medical records and standard computer systems required.

  • Primary care or family medicine experience is preferred; qualified candidates with a strong interest in family medicine are encouraged to apply.

Responsibilities

  • Provide comprehensive primary and preventive care to patients.
  • Evaluate, diagnose, treat, and manage acute and chronic health conditions.
  • Order and interpret appropriate diagnostic testing.
  • Develop individualized treatment and follow-up plans.
  • Educate patients and families about disease prevention, wellness, medications, and treatment plans.
  • Collaborate closely with physicians, nursing staff, specialists, and other members of the interdisciplinary care team.
  • Maintain timely, accurate clinical documentation and support quality and patient-safety initiatives.
  • Help create an exceptional experience for patients and their families.
