About The Position

The APP will work closely with a physiatrist (a physician specializing in physical medicine and rehabilitation) to perform acute care consultations for hospitalized patients. They will evaluate patients to determine the most appropriate level of post-acute care, such as inpatient rehabilitation, skilled nursing facility, home with therapy services, or other rehabilitation settings. The APP will also assist the acute care rehabilitation specialist with inpatient rehabilitation consultations and collaborate with Mary Free Bed Rehabilitation Hospital physiatrists and the multidisciplinary care team to coordinate patient care. This role is primarily at Mary Free Bed Hospital, providing consultation services for patients admitted to Corewell Health's acute care hospital in Grand Rapids, Michigan. The work is generally consultative rather than procedural, with a focus on comprehensive patient assessments, functional evaluations, discharge planning, determining rehabilitation candidacy, and coordinating care with physicians, therapists, case managers, and nursing staff. This is a collaborative inpatient role centered on helping hospitalized patients transition to the rehabilitation setting that best matches their medical and functional needs. The APP is also responsible for demonstrating excellent customer service and standards of behaviors, promoting teamwork, direct communication, and handling confidential information discreetly. They will contribute to quality improvement by identifying problems and seeking solutions, and promoting patient/family satisfaction.

Requirements

  • Certified and licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) by the State of Michigan.
  • Master’s Degree in Nursing or Physician Assistant.
  • 3-5 years of experience with neurologically impaired patients.
  • Current Basic Life Support (BLS) certification.
