About The Position

The Advanced Practice Practitioner (APP) will work closely with a physiatrist to perform acute care consultations for hospitalized patients. The APP will evaluate patients to determine the most appropriate level of post-acute care, such as inpatient rehabilitation, skilled nursing facility, home with therapy services, or other rehabilitation settings. The APP will also assist the acute care rehabilitation specialist with inpatient rehabilitation consultations and collaborate with Mary Free Bed Rehabilitation Hospital physiatrists and the multidisciplinary care team to coordinate patient care. This role primarily operates at Mary Free Bed Hospital while providing consultation services for patients admitted to Corewell Health's acute care hospital in Grand Rapids, Michigan.

Requirements

• Certified and licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) by the State of Michigan.
• Master’s Degree in Nursing or Physician Assistant.
• 3-5 years of experience with neurologically impaired patients.
• Current Basic Life Support (BLS) certification.
• Experience in Hospital medicine.
• Experience in Physical Medicine & Rehabilitation (PM&R).
• Experience in Neurology.
• Experience in Trauma or orthopedic services.
• Comprehensive patient assessments.
• Functional evaluations.
• Discharge planning.
• Determining rehabilitation candidacy.
• Coordinating care with physicians, therapists, case managers, and nursing staff.

Responsibilities

• Perform acute care consultations for hospitalized patients.
