We are searching for a talented Advanced Practice Practitioner to join our Health at Home – Supportive Care Hospice – Palliative Consulting Team.

Requirements

Requires a Masters of Science Degree in Nursing.
Certification from a national accrediting body to practice as a Nurse Practitioner is required.
Current RN license in the Commonwealth of Massachusetts & Rhode Island is required.
A minimum of one-year Nurse Practitioner experience.
Certification in Hospice and Palliative Care required.
Must possess current CPR certification
Certification from a national accrediting body to practice as a Nurse Practitioner is required.
Current RN license in the Commonwealth of Massachusetts is required.
Certification in Hospice and Palliative Care required.
Must possess current CPR certification.

Nice To Haves

Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred.

Responsibilities

Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families.
Perform a wide variety of duties including but not limited to bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc, and serving as a nursing resource for education and consultation to members of the interdisciplinary team.
