Advanced Practice Nurse - Warsaw
About The Position
Katy Trail Community Health is seeking an Advanced Practice Nurse to provide onsite patient care in Warsaw. The role involves assessing patient needs, conducting physical examinations, diagnosing conditions, and developing individualized treatment plans. The nurse will collaborate with a multidisciplinary team, document medical histories, educate patients and families, and participate in team meetings focused on case management and care strategies. The position supports ongoing professional learning and participation in community wellness initiatives. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with no weekends.
