About The Position

As an APN at TriHealth, you’ll join a supportive, patient-centered organization where your clinical expertise is valued, your professional growth is supported, and your work helps strengthen the health of the community. The Nurse Practitioner will provide comprehensive outpatient follow-up care for patients discharged from the hospital after being diagnosed with and treated for a stroke. The role will include assessing recovery, coordinating ongoing care, and addressing the patient’s neurological and overall health needs. A background in neurology is preferred, while experience in gerontology is a valuable plus. The Nurse Practitioner will also evaluate and manage patients living with dementia and Alzheimer’s disease. This is an exclusively outpatient clinic position requiring a four-day workweek, with one weekday off.

Requirements

  • Master's Degree in Nursing
  • Graduate from NP, CNS or midwife program
  • Specialty certification in area of expertise
  • Registered Nurse with APRN Licensure
  • Demonstrated expertise in area of clinical specialization
  • Professional membership in specialty nursing organization required within one year of start date
  • 3-4 years experience Clinical in Nursing
  • Nursing practice in geriatric, occupational health, or other applicable setting.

Nice To Haves

  • Background in neurology is preferred
  • Experience in gerontology is a valuable plus

Responsibilities

  • Collaborates with the interdisciplinary healthcare team, patients, and families to diagnose, prioritize problems, implement interventions, and coordinate referrals while promoting teamwide collaboration.
  • Assists team members, patients, and families in selecting therapies by integrating considerations of cost, quality, and benefit, and partners with other disciplines in teaching, consultation, management, and research to improve nursing practice and the healthcare environment.
  • Provides effective leadership by analyzing cost–benefit considerations, designing innovative, resource‑efficient care solutions, and guiding the development and application of evidence‑based practice (EBP) guidelines and consensus protocols.
  • Participates in healthcare decision‑making at system, local, state, and national levels to improve patient outcomes, and serves in leadership roles on committees, councils, and administrative teams.
  • Utilizes, participates in, and disseminates research, EBP, and quality‑improvement projects; evaluates current practice based on research; and mentors nurses through activities such as Journal Club and appraisal of clinical literature.
  • Provides education to patients, families, nurses, students, and other healthcare professionals; mentors and precepts staff; promotes nursing research and EBP; and collaborates in assessing, developing, implementing, and evaluating educational needs.
  • Ensures ethical and legal nursing practice; provides safe, compassionate, accountable care; advocates for patients’ wishes and end‑of‑life decisions; and independently conducts comprehensive assessments, prescribes medications and devices within scope, orders and interprets tests, and collaborates with physicians to implement comprehensive care plans.
