About The Position

The Advanced Practice Nurse will be responsible for conducting patient visits in accordance with regulations, communicating findings to the interdisciplinary team, facilitating access to appropriate support models of care, and consulting with team members on patient care plans. This role also involves preparing and maintaining accurate patient records and assisting in the development of clinical practice guidelines. Collaboration with patients, caregivers, medical directors, and the interdisciplinary team is essential.

Requirements

  • Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Nurse Practitioner's license in the state of operations.
  • 1+ years experience working within Family Practice, General Practice, Geriatric care or related field.
  • A valid driver's license and a reliable source of transportation.
  • Willingness to travel.

  • Experience in a nursing home, hospital, assisted living or acute rehab space.

Responsibilities

  • Conduct visits to patients in accordance with local, state and federal regulations.
  • Communicate visit findings to designated members of the interdisciplinary team.
  • Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
  • Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
  • Prepare and maintain accurate patient records, charts and documents.
  • Assist in development of clinical practice guidelines/standards in support of quality care as needed.
  • Ensure collaboration with patients, caregivers, medical directors and members of IDT.
