About The Position

This is a Per Diem Advanced Practice Nurse position for Bridge Care, with opportunities in Livingston NJ, Madison NJ, and Hanover NJ. The role involves conducting patient visits, communicating findings to the interdisciplinary team, facilitating access to support models of care, consulting on patient care plans, maintaining patient records, assisting in the development of clinical practice guidelines, and collaborating with patients, caregivers, medical directors, and the IDT.

Requirements

Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Nurse Practitioner's license in the state of operations
1+ years' experience working within Family Practice, General Practice, Geriatric care or related field
A valid driver's license and a reliable source of transportation
Willingness to travel

Responsibilities

Conduct visits to patients in accordance with local, state and federal regulations.
Communicate visit findings to designated members of the interdisciplinary team.
Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
Prepare and maintain accurate patient records, charts and documents.
Assist in development of clinical practice guidelines/standards in support of quality care as needed.
Ensure collaboration with patients, caregivers, medical directors and members of IDT.
