The Advanced Practice Clinician will provide and coordinate primary care to SCAN members to achieve the Company’s mission to keep seniors healthy and independent. This field-based role covers San Bernardino, CA. The clinician will provide care to an assigned panel of members in their place of residence, typically in senior living communities or skilled nursing facilities. Responsibilities include conducting exams and assessments, occasional urgent visits, telephonic triage, administering vaccines, ordering and interpreting diagnostic tests, initiating goals of care conversations, and chronic disease management. The role involves identifying patient needs, developing care plans, collaborating with case management, managing visitation schedules, engaging with Primary Care Physicians, connecting with specialists and other providers for continuity of care, monitoring clinical quality gaps, documenting visits, serving as a direct resource for members and families, participating in sales team events, initiating virtual health visits, and participating in an after-hours on-call program. The position requires maintaining professional knowledge, contributing to team goals, and supporting SCAN's vision and goals. Employees may be eligible for differential pay. This is a patient-facing role.
