About The Position

Alignment Health is seeking an Advanced Practice Clinician (APC) to join a transformative program focused on providing personalized, high-quality clinical and home-based patient care to seniors and those with complex/chronic needs. The APC will be responsible for assessing, developing, and coordinating care options tailored to members, typically aged 65 and older. This role involves home and virtual visits ranging from 45 to 60 minutes, allowing for focused, compassionate care during initial visits, annual check-ups, or when additional support is needed for medical and social challenges. The position adheres to Alignment Health’s policies and state/federal regulations, aiming for optimal health outcomes and cost-effective solutions.

Requirements

Master’s degree from an accredited NP Program or PA program
Active California state Nurse Practitioner or Physician Assistant license
Active RN License and Furnishing number (Must, upon hire) - NP only
Active Nurse Practitioner Board Certification/Physician Assistant Certification
NPI Number
DEA
Valid BLS
Valid California driver license and current automobile insurance

Nice To Haves

One (1) year of prior clinical or home care experience.
Previous EMR experience preferred.
Experience in care of older adult (geriatric) patients preferred.
