About The Position

Alignment Health is seeking an Advanced Practice Clinician (APC) to join a transformative program focused on providing personalized, home-based care to seniors and chronically ill patients. This role involves conducting home and virtual visits, ranging from 45 to 60 minutes, to deliver high-quality clinical care, assess patient needs, develop care plans, and coordinate services. The APC will ensure optimal health outcomes and cost-effective solutions while adhering to company policies and regulations. This is a future opportunity and/or an upcoming class.

Requirements

Master’s degree from an accredited NP Program or PA program
Active California state Nurse Practitioner or Physician Assistant license
Active RN License and Furnishing number (Must, upon hire) NP only
Active Nurse Practitioner Board Certification/Physician Assistant Certification
NPI Number, DEA, Valid BLS
Valid California driver license and current automobile insurance

Nice To Haves

One (1) year of prior clinical or home care experience.
Previous EMR experience preferred.
Experience in care of older adult (geriatric) patients preferred.

Responsibilities

Ensure the delivery of high-quality clinical and home-based patient care.
Promote optimal health outcomes and provide cost-effective solutions.
Adhere to Alignment Health’s policies, as well as state and federal regulations.
Assess, develop, and coordinate care options tailored to the needs of members, typically 65 and older.
Conduct home and virtual visits ranging from 45 to 60 minutes.
Provide focused, compassionate care for initial visits, annual check-ups, or when additional support is needed to address medical and social challenges.
