About The Position

This role provides diagnostic, preventative, and therapeutic health services to patients and family members, functioning either independently or with physician supervision. The clinician will secure health histories, perform physical exams, order and interpret diagnostic tests, and make critical evaluations. Responsibilities include making decisions about medical care needs, providing direct medical care for illness or disability, managing chronically ill patients, and collaborating with physicians on complex medical problems. The position also involves participating with other professionals in health care services, evaluating treatment plans, assessing total health care needs, and providing medical care and preventive services to healthy individuals. Teaching patients and families about health maintenance and improvement, maintaining patient records, and participating in Quality Assurance (QA) and Performance Improvement (PI) processes are also key duties. The role includes providing emergency services and crisis intervention.

Requirements

  • Applicant must have a valid unrestricted insurable driver's license.
  • Resume and references are required.
  • Nurse Practitioner: Completion of an Advanced Nurse Practitioner School in any of the following areas: Accredited Family Nurse Practitioner, Accredited Adult Nurse Practitioner, Accredited Adult-Gerontology.
  • Nurse Practitioner: Active unrestricted Advanced Nurse Practitioner license in any U.S. State or Territory in any of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Adult-Gerontology.
  • Nurse Practitioner: National Certification from the American Academy of Nurse Practitioners.
  • Nurse Practitioner: Active unrestricted Nurse Practitioner license from any U.S. State or Territory.
  • Physician Assistant: Completion of an accredited Physician Assistant Program.
  • Physician Assistant: Board Certification by the National Commission on Certification of Physician Assistants.
  • Physician Assistant: Active, unrestricted Physician Assistant License from any U.S. State or Territory.

Responsibilities

  • Provides diagnostic, preventative, and therapeutic health services to patients and family members with the ability to function as an independent practitioner or with appropriate physician supervision.
  • Secures a health and developmental history from the patient or parent and perform a detailed physical exam, records findings and makes critical evaluations.
  • Performs or requests special screening and developmental tests and diagnostic tests and procedure and interprets the results. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems.
  • Makes decisions concerning medical care needs of patients which may involve various treatment modalities, prescribed medications, consultation and referral when appropriate.
  • Provides direct and independent medical care in the presence of illness or disability in order to maintain life, provide comfort, reduce distress and enhance coping ability.
  • Provides surveillance or medical practices applied to stabilize chronically ill persons; adjusts treatments within established standing orders recognizing when to refer the patient to the physician or other health team members.
  • Works collaboratively with physician in management of selected complex medical problems.
  • Participates with other professionals and agencies involved in providing health care and services required to include immunization and disease control, environmental health and health education.
  • Evaluates the medical aspects of treatment plans periodically, recognize when reassessment by a physician or other health professionals is appropriate. Evaluates total health care needs of patients and develops or initiate plans to meet these needs.
  • Provides medical care and preventive services to healthy individuals, including guidance in nutrition, common illnesses, accidents and immunizations.
  • Maintains knowledge and proficiency in medical practices through continuing education, staff meetings, workshops, and home study.
  • Teaches patients and family member’s ways to maintain or improve their health status.
  • Maintains records and reports on patients, personal activities and other required information for use in program operation, quality assurance and other purposes.
  • Participates in Quality Assurance (QA) and Performance Improvement (PI) review processes.
  • Provides full range of emergency services or crisis intervention including lifesaving emergency procedures in order to stabilize a patient sufficiently until a higher level of care is available.
